REGISTRATION & WAIVER OF LIABILITY

 

 BY SIGNING THIS DOCUMENT, YOU WILL WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE OR CLAIM COMPENSATION

 

PLEASE READ CAREFULLY!

 

IN CONSIDERATION OF the Canadian Junior Golf Association (the "CJGA") allowing the participant signatory below ("I", “my”, “myself”) to participate in CJGA events (the “Activities”), and for other good and valuable consideration, I agree to the CJGA’s terms and conditions set forth in this agreement (the “Agreement”) as follows:

A.             Registration and Membership

1.              I understand that the acceptance of my application by the CJGA is conditional on my agreement to all provision of this Agreement. Without my consent to this Agreement, my application is invalid. I understand that my CJGA membership application is subject to acceptance or rejection by CJGA.

2.              I agree to abide by any regulations laid down by the CJGA, Golf Canada and USGA Code of Conducts. I agree to abide by the rules and regulations of my CJGA membership at all times, including the CJGA Policies and Procedures, as well as any and all other guidelines, protocols, rules and regulations of the CJGA. It is the responsibility of the participant and the parents or guardian(s) of the participant to read and understand the CJGA Policies and Procedures, located online on the CJGA website at the url: https://cjga.com/cjga-policies-procedures-2/ (the “CJGA Policies and Procedures”). I verify that I am qualified by the CJGA rules of eligibility as contemplated by the CJGA Policies and Procedures

3.              I agree to make all reasonable efforts to study the Rules of Golf of the Royal Canadian Golf Association.

4.              I understand that the CJGA has a NO REFUND POLICY. Membership fees are non-refundable. Memberships are yearly, and expire on December 31 of each given year.  Any credits expire in one (1) year from the date the credit was given. When credits are given and used for tournaments, said credit will expire if the player to which the credit was given subsequently withdraws from the tournament entered.

5.              I acknowledge that in the event that my breach of CJGA’s rules results in the suspension of my membership by the CJGA, the CJGA may disclose my suspension to Golf Canada and all provincial associations at its discretion.

6.              I understand that my failure to follow the Code of Conduct and Tournament Procedures of the CJGA can result in the revocation of my membership and/or tournament registrations without refund of any associated fees.

 

B.             Media

1.              I grant the CJGA the right to photograph, videotape and/or record me and to use my name, face, likeness, voice and appearance in connection with exhibitions, publicity, advertising and promotional materials without reservation, limitation or compensation. 

2.              I grant CJGA the right to publish any and all photos taken of myself at CJGA associated events by the CJGA or any of its representatives. Said photographs, videotapes and/or recordings of me may be used in any brochures, announcements or publicity releases by the CJGA, its representatives or its sponsors, whether in print of online.


I, the participant, and I, the parent/guardian, accepting my own responsibility, hereby release the Host Facilities at which events are conducted, the Canadian Junior Golf Association, its officers, directors, employees, agents, sponsors, and principals from any and all liability for any event or consequence whatsoever in any way arising out of or relating to members' participation in any Canadian Junior Golf Association event. In case of emergency during a Canadian Junior Golf Association tournament, I, the participant, and I, the parent/guardian, 
authorize a qualified medical doctor or Emergency medical services agent, also known as ambulance services or paramedic services, to provide urgent pre-hospital treatment and stabilisation for serious illness and injuries and transport to definitive care and to take all necessary measures in the treatment of myself, the participant, friend or spectator related in some way to the participant

I, the participant, and I, the parent/guardian, also understand the CJGA has a NO REFUND POLICY. Membership fees are non-refundable and membership expires on December 31 of given year.  Any credits expire in one year from the credit given. When credits are given and used for tournaments, if a player then withdraws again the tournament that they entered, the credit then expires. Once a player has applied, a player should note that acceptance is upon application made unless otherwise notified. 

I, the participant, and I, the parent/guardian, further agree that all photos taken by the Canadian Junior Golf Association or any of its representatives, of myself may be used in any or all brochures, announcements or publicity releases by the Canadian Junior Golf Association or representatives or sponsors of the Canadian Junior Golf Association.

I, the participant, and I, the parent/guardian, further agree that any breakage of the rules that results in a suspension by the Canadian Junior Golf Association may be made known to Golf Canada and all Provincial Associations. I, the participant, and I, the parent/guardian, further agree that all e-mail addresses submitted by members or family members may be used by the CJGA for the purposes of its membership.

This statement must be agreed to or the application is invalid. I, the participant, and I, the parent/guardian, understand that by becoming a member and by participating in CJGA events, we consent to the disclosure of information concerning performance and status of membership to the news media, other golf associations and organizations interested in the game of golf.

I, the participant, and I, the parent/guardian, accepting my own responsibility, further hereby Expressly Consent to receive emails from the Canadian Junior Golf Association and its partners. This Express Consent is in accordance with new Canada's Anti-Spam Legislation (CASL) which came into force on July 1, 2014.

I, the participant, and I, the parent/guardian, agree that I, the participant, and I, the parent/guardian, have the legal authority to complete this application and have the authorized right to use the credit card being used for registration.

I, the participant, and I, the parent/guardian, further agree to the use of any and all private information provided for the use of all processes used by the Canadian Junior Golf Association for the purposes of registration, news releases, communication with institutions of education, online scoring and results and any and all other uses to the Canadian Junior Golf Association uses in order to carry on business.

I, the participant, and I, the parent/guardian, acknowledge that the Canadian Junior Golf Association has made all attempts to ensure that its online registration system is secure. However, although the Canadian Junior Golf Association has made all reasonable attempts to prevent unauthorized access to any data, the Canadian Junior Golf Association makes no warranty that the Participants data will be completely secure against unauthorized access. I, the participant, and I, the parent/guardian, accepting my own responsibility, hereby release the Host Facilities at which events are conducted, the Canadian Junior Golf Association, its officers, directors, employees, agents, sponsors, representatives and principals from any and all liability, claims, damages, losses or consequence whatsoever in any way arising out of or relating to members' use or in connection with any services provided by the Canadian Junior Golf Association including its online registration system.

The Canadian Junior Golf Association ("CJGA") is committed to respecting the privacy of our members, their families and our employees by adhering to the privacy principles set forth in the Personal Information Protection and Electronic Documents Act ("PIPED"). 

The CJGA's Privacy Policy adheres to the ten principles of the PIPED Act, and these principles can be obtained from our offices in written form, or it may be found on our website. If you choose to provide personal information to the CJGA, we assume that you consent to the collection, use and disclosure of your personal information as outlined in CJGA's Privacy Policy or otherwise at the time of collection, use or disclosure (

Disclaimer

I, the participant, and I, the parent/guardian, accepting my own responsibility, hereby release the Canadian Junior Golf Association, its officers, directors, employees, agents, sponsors, principals, and host facilities at which events are conducted from any and all liability for any event or consequence whatsoever in any way arising out of or relating to members' participation in any Canadian Junior Golf Association event.

I, the participant, and I, the parent/guardian, also understand that upon acceptance into a tournament there are NO CASH REFUNDS. A credit may be applied if a player withdraws outside of the 8 day withdrawal time (see Withdrawals on the CJGA Website at https://cjga.com/about-cjga/cjga-policies-procedures/). Acceptance is upon application unless otherwise notified. 

 

I have had sufficient opportunity to read this entire document. I have read and understood it and I agree to be bound by its terms by checking each box required in registration.
